Oracle SQL High-Performance Tuning - Guy Harrison is excellent. It's primary focus is application vice server tuning. He dedicates an entire chapter to single table access tuning and another to multiple table access tuning. Some of our more 'seasoned' DBAs have even learned a few things from it.
