 I had exactly the same error. In my case it was because my Xserver was set to use more than 256 colours - this was a known Oracle bug with the eXceed server I was using (NB. Oracle bug, not eXceed bug). Oracle emailed me a long document of problems and workarounds for these toolkit II type errors with Server Manager, ask them for a copy.
