You can look at V$DATABASE_INCARNATION, but it doesn't track all history and there of course are things as human you can do that would appear here too.

Gurus,
We have Oracle 12.1.0.2 Active Data Guard. Upper level management recently requested a report that will show switch overs and failovers, date and time, from primary to secondary instance name, etc.

If the document exists that explains/shows how to accomplish this I have not been able to locate it. The only thing I have come up with is getting information from the dataguard log which is very cumbersome.

Does anyone have a more elegant solution?
