Hi Andrew,
Why not trap the error:
ALTER SYSTEM SET EVENTS='48189 TRACE NAME ERRORSTACK FOREVER, LEVEL 16'; When the error occurs, it will emit a trace file which will, with any luck, contain the information you need.
