Here's what I do...
I generally would not apply one-off patches for a problem we have not encountered. (For exceptions to this, reasoning would be documented.)  Once a one-off has been installed, it needs to be maintained -- if it is not rolled into a PSU then a merge may be required for future updates. The one-off would also need to be installed for all other databases in the organization running the same application as the one which generated the problem, and in some cases perhaps it should be installed on all databases in the company running other apps.

I will review the release notes for each PSU. Generally I would prefer to stay on top of installing these, but if we are tight on resources (DBA, testing, etc) and nothing in the PSU seems to apply to our particular architecture, I may skip a PSU. It should be documented that we did review the PSU and made this assessment.
