This is RAC so I assume that RAC configuration is moved. The details are still moot but if you need to change IP's on your RAC - there is much more to do. Assuming that you currently have your physical standby configured as RAC, you will need to reconfigure VIPs and public interfaces, reconfigure interconnect, ONS daemons, listeners, tns descriptors, possibly remote/local listener registration.

If you are changing hostnames, than the most straightforward way is to reinstall your clusterware and RAC. Alternatively, add new nodes and remove old ones if that's feasible. Those are "standard" ways I would say. There is another way - it's a bit more tricky and might not be easy to follow if you are not very comfortable with CRS administration (I blogged about it some time ago if you are interested).
