I am not sure Oracle would support MTS in OA. For sure, MTS is not advised in either a batch-mode environment or in an OLTP environment where the queries are not short, one-liners. Although one may argue that the Forms mode and Self-Service of OA generate short queries, the Concurrent Managers certainly do not.

So I would ask this question: Is there a separate TNS handle for the CM that goes to dedicated servers and a separate handle that goes from the Forms/Self-service layer? If not, then it is very possible that your dead server is because of a misconfigured configuration.
