Another possibility is that transactions are queueing on simple row updates
on IOTs (i.e. something that would
give a mode 6 TX lock for heap table
gives a mode 4 on IOTs)

There are certainly cases where TX enqueues can wait on mode 4, that is, where REQUEST is mode 4.
