It's documented that, effective 9iR2, the second digit now represents the database version and the third digit represents the app server version.
Upgrading the database (known as migration) can be done independantly of upgrading the software. Unless there is a specific reason for doing so otherwise, I generally recommend installing the latest version into a separate Oracle Home and migrating existing databases in a planned fashion.
So my suggestion - download the latest, and install that. Better yet, sign up for TechTracks and get the CDs shipped.
