Thanks to both yourself and Niall for the suggestions, I am starting
to suspect the software (Omnis7) as I have also had problems recently
with a select from DUAL for the next sequence number, ie. in 20000
consecutive inserts it failed once with a row count of zero ... this
occurs occasionally even in our test database.
I have some serious investigations to do )
