The Instance is the shared memory area (SGA) plus the background processes which manage the database (which is the physical collection of control files, data files and redo logs). Accordingly, until you get into parallel server, one database is always managed by one Instance, and an Instance can only ever manage one database.
In parallel server, whilst it remains true that each individual Instance can only manage the one database, it becomes possible for the one database to be managed by multiple Instances.
