Check to see if the inso filter is grabbing a lot of cpu as well.  Even if
it isn't it may be worth it to manually pass this particular document
through the ctxhx executable to see if it creates the desired output (html
or text) etc...
Is ctx_user_index_errors containing anything or is dr$index_error seemingly
adding extents while ctxsrv is spinning?
