Backup & Recovery Procedures

Ho often do folk actually run a test of their backup & recovery procedures - not to mention disaster recovery plan? Also what scenarios do people test. I have my own plans & ideas but what I'm after is some sort of professional consensus as to what is an adequately tested backup & recovery strategy - cos you can be sure that you asked when something goes wrong - why didn't you test what would happen if a nuclear device were detonated over your place of work?
