Brand new database crashes apps

I have been successfully running one 8i database on my server, but just recently I needed another one so I used Database Configuration Assistant to create it. Everything was going fine until 90% of the way through creating the database an error came up (ORA-12631: Username Retrieval Failed). I hit Ignore and the creation process froze right there. From that point on, I was able to start the service for this new database, but many things still give the error mentioned above or simply freeze the program I am using. I can't even delete the database because Config Assistant freezes in the same way as all the other Oracle tools.

I am wondering if anyone has seen this before and if so, why does it happen and how I can fix it.
