I suppose you're on Windows NT 4.0 SP5 or later? If on SP5 did you earlier have a SP3 installed? Unfortunately, you need to go through SP3 before installing SP5, some corrections that came with SP3 were forgotten in SP4 and SP5 (not sure about SP6 but AFAIK OAS 4.0.8.1 isn't supported with SP6).
