Cannot create database on NT for 8.1.5

I have little experience for the Oracle for NT. Recently, I have installed Oracle for NT 8.1.5.0.0. When I tried to create the new database through the Oracle Database Configuration Assistant, the errors occurred:

    TNS-12560: TNS:protocol adapter error.

I tried to run svrmgrl in DOS Command Prompt. Same error occurred.

When I set both environment variables ORACLE_SID and LOCAL to test_db, following error occurred:

  ORA-03113: End-of-file on communication channel.

I wonder what happens in the case. Can anyone help me? I have configured tnsnames.ora, sqlnet.ora and listener.ora. (I wonder whether the Server Manager needs such files.) NT machine is of version 4.0 with SP3, 80MB RAM. I have increased to 128 MB RAM but the result is same.
