Sounds like there may be a problem with the virtual path spec in your PLSQL
cartridge. In the OAS Manager, look under:
This should list one of more virtual paths that can activate this cartridge. These are case sensitive. In my case, I have a path like "/dev1/plsql". If my server name is webdev1, I'd test this out by using: http://webdev1.x.y.z/dev1/plsql/oas_public.htp.p?cbuf=success
If this works, your browser should display 'success', and your application status should show that it's up.
