application server 4.0.7 - cartridges missing?

I have used Oracle application (web) server v3 for some time and have recently installed 4.0.7 and I am having some difficulty setting up my first v4 service.

In v3 it is straightforward to set up a DAD, listener, web request broker and PL/SQL agent. In v4 I quickly set up the listener, now I am stuck.

From OASTools.html
I click on Utilities ... Install ... PLSQL toolkit and I get the error: "The document contained no data. Try again later."

Clicking on 'DB Access Descriptor" from oasmgr.html gives the same error (document contained no data).

I open Applications ... Server status ... cartridges and the only one present is 'web server status monitor'. Where is the PL/SQL cartridge?
The admin guide says (p. 12-2): "OAS comes with the following cartridges that you can use ... PL/SQL cartridge".

Any help appreciated.
