ORA-01200 on RAID 5

We have started using 8.0.5 and have seen a lot of the following error:

ORA-01200 Actual filesize xxxxxxx is smaller than correct size yyyyyyy blocks.

The error occurs during a CREATE TABLESPACE statement. It has happened on both Dell and Compaq servers now, so I have ruled out a hardware issue. All three servers are NT 4 with SP3 and nothing appears in the event log.

Any ideas?
