Where I work, we were having problems with the cost based optimizer doing tablescans when there were indexes available. We made sure that we had current statistics for the tables involved, but nothing changed. We then set the 'optimizer_mode' parameter to 'first_rows' in the init.ora file. This did the trick and the optimizer started to use the proper index. In talking to oracle, they said there might be a bug in the optimizer when the 'optimizer_mode' is set to 'choose' (the default value). Hope this helps.
