You should be aware of the following:

• An exception is thrown if you try to open a non-existing file in append mode
  • you have to open it in write mode in order to create it.
• If you try to open an already open file in write mode the file is sometimes truncated.
• Remember to set up utl_file_dir in your init.ora file. Be aware, that the directories shall match EXACTLY the files you're working with (case sensitive even if you're running on an OS with case insensitive file names, full/relative path etc).

I've logged these "features" with Oracle Support.
