I have seen this error several times with the NT config you describe but without oracle. They appeared on a heavily loaded network with NT being unable to load any application program. The only chance was to reboot the PC.
It seems to be an NT problem. Backing out of the FP5 or upgrading to NT4.0 *may* remove the problem.
