I had EXACTLY the same problem with a Gateway P-200 with a 3com PCI 100/10 card, NT server 3.51, SP4. The beast would just go AWOL, and over IP, netBeui, or appletalk, it just wasn't accessible. IF you logged in on the console, everything looked OK, and only a reboot would fix it.
This was after my standard procedure of setting up the card BEFORE installing NT. Boot up from floppy in dos 6.x, set card IRQ, I/O, network prefs & topology type from the 3com utility. Then go in to BIOS and disable any power management. Then install NT 3x or 4.
After a few days of sweating bullets, I found that there was a system device driver for the 3com bus master card that was set to manual startup. When checked on from Control, Device, it was not started. Set the startup for it to Automatic, rebooted, all problems vanished.
