dup_val_on_index _is_ unique constraint violation (code ORA-00001) see "Predefined exceptions" in PL/SQL User's Guide and Reference: http://download-west.oracle.com/docs/cd/A87860_01/doc/appdev.817/a77069/06_errs.htm#784
