I am getting a similar problem. I installed the 'isqlplus' patch 9.2.0.1 with a newer date by 5 months and I still have the problem (on Windows XP Pentium 4 machine). The log looks like:
[Mon Apr 07 14:01:32 2003] [notice] FastCGI: process manager initialized
[Mon Apr 07 14:01:33 2003] [warn] FastCGI: server "d:/program
files/orawin/bin/isqlplus" started (pid 3132)
[Mon Apr 07 14:01:34 2003] [warn] FastCGI: server "d:/program
files/orawin/bin/isqlplus" (pid 3132) terminated with exit with status '3373160'
..
followed by
[Mon Apr 07 14:02:27 2003] [warn] FastCGI: server "d:/program
files/orawin/bin/isqlplus" can't be restarted:exceeded max restarts
[Mon Apr 07 14:02:27 2003] [warn] FastCGI: server "d:/program
files/orawin/bin/isqlplus" can't be restarted:exceeded max restarts

ad infinitum, growing to several gigs very quickly until I shut down apache http server.
