I would further add:
6. Have they done this before?
7. Are they doing it now? If so, can you get references to call? 7a. No one gives bad references, but call the refs. and ask if they have had the hosting company provide 1,2,&3 for them. 8. Most of your larger Co-Los (like Globix, Exodus, etc.) have and will do this. Call and get pricing and lead times as well.
