Forms modules

I am having trouble trying to create a form (Forms 4.5) for a newly created table under Oracle 7. The server is unable to locate a module file for the form. I looked for some files on the Unix host that Forms is running on but could not find any of the default extensions for Forms. Do I need a template module file to create an inititial form for a table? If so, does anyone have one that I can use?
