You may be right about the extension.  But assuming that you are using
SQL*Menu5.0 with Forms 3.0, you need not give the file extension:
Here's the correct syntax:
f45gen <module_name> <username>/<password> module_type=menu upgrade=yes.
example:
f45gen main_menu scott/tiger module_type=menu upgrade=yes
The above should work unless your SQL*Menu version is different(I definitely was able to convert for 5.0). This will create both .mmb and .mmx files for forms4.5 use.
