Form Buider 6.0 & Oracle 7.3.4

Hi,
I have this problem:
The insert or update operations in form converted from 4.5 and called by call_form from
another form converted from 4.5 to 6.0 cause an incredible result:

FRM-40509 Oracle error: unable to update record ... Follow by Transaction OK's message but the operation, in effect, is terminated
unsuccessfully.

The Display error operation results:
Sql statement error: no sense characters Error: ORA-01403 no data found.

This "special effect" appears randomly but frequently and with form developed by 6.0
too. The oracle patches hasn't resolved this problem.

The question is: any other has encountered and solved this problem.
