I can recommend two companies, both of which have offices in the Chicago area. Aris is one (www.aris.com). We sent a DBA to one of their courses earlier this year, and I was given good a very good report. Another company to look at is TUSC (www.tusc.com). They are very good, and I believe they offer some training.
