problems while working with generated modules

We've encoutered a problem while working with some of generated for Oracle Web Server PL/SQL modules. First screen : module_name$.startup works always properly in browser, but in some modules, not in all of them, an attempt to add a new record, execute query or update data causes error shown in DrWatson pop_up dialog :

wrks.exe error Exception : access violation (0xc0000005), Address 0x01f09599

What's that? Is wrks.exe a part of OAS services, or at all sth connected with Oracle or maybe a part of WindowsNT Server we are using. How to handle this error? What's the reason of it?
Any help would be apreciated.
