Sending mails via froms

Here's the thing. I need to create a form (using either form4.5 or 6.0) where it is possilble to select one or more files from an lov and also select an email address from an lov. Here after i press a button ("send email") and an e-mail is mailed of with the file(s) attached. Is this possible and if so how do you do it.
i konw how to make the form I need to know how to automatically send the e-mail with file enclosed.
