Forms 4.5 16 bit with Oracle 8.0.4 Database

I use a Personal Oracle 8 Database with Developer 2.1 (FORMS 5, REPORTS 3, SQL*Plus 3.3) without problems.

I install Forms4.5 (16 bits), but i can't connect to database:

ORA-09354: Windows 32-bit Two Task driver: ORACLE task unexpectedly died

Help me please

What I need to do to connect Forms 4.5 16 bits to Oracle 8 Database ???
