I had the same problem a few days ago and received a mail from David J. DeWolfe (sxdjd_at_java.sois.alaska.edu). He told me the following which works great for me: Check out the upgrade optin ti the (forms 4.5) genform45 command. Basically it will convert a 3.x form to a 4.5 one.
