I don't think this is possible since you'll have to install APPL_TOP on the FORM server for the FORM portion of the APPL_TOP and you only have software for UNIX platform (I would imagine)

On R10.7 Prod 16.1 you could've done something like this with THIN client install option, having
DB, CONCSUB on UNIX and FORMS on SHARED FORM SERVER, but with R11 it's different story now
FORMS have been moved back to APPL_TOP

Any way I would call Oracle on this ONE.
