Prompts in Forms 4.5

Hi everyone,

I wondered if anyone could tell me a simple way of displaying a modal dialog box which will let the user either enter a value or choose one from a list?

Is it possible to do this without creating a new form and passing the value back to the calling form as a parameter, for example by creating a new window and just displaying it when required? Any advice would be appreciated. Thanks in advance,
