the Oracle Messages and Codes Manuals says that the error occurs when e.g.
the select statement returns more columns (!) than what has to be inserted.
I checked your example but it looks ok. Maybe you can take a look at that
your self again.
When you use indexes and you try to insert a row that already exists the
error message is different. (Something like "Unique constraint violated".)
That not very much information but maybe it helps you.
