Losing/missing data during INSERT

Hi,

I am building a form which inserts data into a table from another based on certain conditions(one being a date). When I insert the data I consistently seem to lose/miss 15 records irrespective of the date that I am copying data for. I lose the 15 records whether I try to do the insert in SQL*Plus or in forms. Could someone help me out with this.
