Check the coordinate system parameters in your forms module property settings. This type of thing is a common problem if you are using character based coordinates. I think it is similarly a problem if you use real coordinates with the pixel setting.

I suggest setting the coordinate system in each form to real with point, centimeter, or inch settings.
