How to coordinate user passwords among instances

We have an environment in which users may connect to one of several instances
(for training, testing, production). If they change their database password, they
need to change it in each instance separately. We would like to put in a system
so they can change their password once and have it apply to all the instances in
which they have accounts.

Does anyone have a system to do this?

What do you all do to work around this coordination problem?
