oracle forms 4.5 problem helpneede

I have two problems in oracle forms 4.5 . The problem is

I have four tables, Patient table,Doctor table,location, and history table.I have three screens.In my 1st screen which is 'checkin check out' I have three fields in it Doctor's id, Doctor's name and location second block comes from the patient table which has two fields patient no, and patient name.My problem is I want to assign a new patient to that doctor,say Doctor no. 1002 will have a new patient named nina with patient id 2002 (This patient id and name does not exist in the database) and on pressing OK it should update this change in my database. 2) Problem two Second screen is the find screen.It has three fields patient no, patient name and search field. My problem is if i type say
'S' in the patient name and then go to 'Search' it should display all
the names (a new window should pop up) starting with 's'.Or if i type
'BOB' it should display alL the names starting with 'BOB'.

Your help will be appreciated.
