Thanks for your update, the bug 8286901 is fixed in 11.2, so i think the most possible culprit is the bug 13341274.because there is no patch ready for bug 13341274 and you don't have downtime to rebuild the freepool of lob index segment, i

What's the business impact for the slow insert? if the business is seriously impact, it may be possible for you to apply downtime to apply the workaround suggest in the note 365156.1.
