i have created a form using form builder
compliled it on putty ,it successfully created bt i dont know how to attach it to the apllication r12

Read the Forum Guide.
Place your fmx in $PROD_TOP/forms/US.
1. Create Form Function.
2. Attach that to a menu, which is attached to your responsibility.
