looking for old Datamation poster on Normalizing

I used to have a useful poster that I probably got as a freebie from Datamation magazine about 20 years ago. It nicely explained the various Normal Forms and I think it used some sort of dog breeding example. I remember seeing it posted in other offices, so it was fairly popular. Some of you other geezers might remember it.

Does anyone know where to get a copy, or would you be willing to scan yours?
