print records

I have a table that contains data such as name, phone, address, email, date, etc.. I need to print the records from this table to a prefabricated  certificate. The field names must align to the pre-printed format on the certificate, for example:

Name: ______________ Date: __________

some text here

Address: _____________________________

some text here.... Phone: __________ Email: ______________

Please suggest the best way to solve this problem. Thank you very much for your help
