The standards spec is at the site of the standards committees at
sqlstandards.org; deep in that site is
http://sqlstandards.org/SC32/WG3/Progression_Documents/Informal_working_drafts/
which has the SQL:2003 drafts in .pdf form. Each section has some
description of SQLSTATE codes.
