Questions and Answers in the database

I'd like to see some different approaches to modeling this in a relational database. A survey may contain 1 or more questions (probably never more than fifty) and the answers can take various forms. Some answers are simple Y/N others are one or more values drawn from a pick list, others are free form text answers. So both the number of answers and their values may change for each question.
