Most university libraries would have the CACM (probably on microfilm/microfiche).

I have an aging copy of "Readings in Database Systems", edited by Michael Stonebraker, 1988, Morgan Kaufman, ISBN 0-934613-65-6 which has Dr. Codd's paper beginning on p5.
