Which software can capture all screen shots and composit them into a demo?

I need find of which software is the best one you ever used that can capture screen shots and then let you add some comments, then combine them into a demo.

The demo can be a set of web pages or single PDF file, which can be used for training your end users or for sales team.

Say, you created a new database, you need show your user how to use it, click which menu, select which option etc. You better have a sequence of screen shots then you can published on your web site or print out ...

You will need to add some explanations, warnings or other text message to the demo.
