Most likely it is caused by an entry in tnsnames.ora to be missing or otherwise incorrect.
Does this error occur when trying to connect to the DB or to the forms server itself?
What OS?
What OS version?
What Oracle DB version?
Where does the DB reside relative to the Forms Server & Client?
Do you get the error trying to build a form or use a form?
The minor "details" above matter to getting a real fix.
My crystal ball is in for repair this week.
